It identifies the parties involved in the case and the type of pleading that is being filed. The cover sheet is required for all pleadings in South Dakota domestic relations cases, including motions, petitions, orders to show cause, and answers. The cover sheet must include the names and contact information of the parties, attorneys, and guardians ad item; the case number; the type of pleading; and the date the pleading was filed. There are two types of South Dakota Domestic Relations Pleadings Cover Sheet: one for initial filings, and one for subsequent filings.
