The Pennsylvania Certification of Acknowledgment of Paternity is a legal document that is used to formally recognize a child’s legal father. This document is used to establish paternity when the father’s name was not included on the child’s birth certificate. It should be completed and notarized by both the mother and the father, and then filed with the Pennsylvania Department of Health Bureau of Vital Records. There are two types of Pennsylvania Certification of Acknowledgment of Paternity. The first is the Voluntary Acknowledgment of Paternity, which is used when both parents are in agreement about the paternity of a child. The second is the Disputed Acknowledgment of Paternity, which is used when one or both parents are not in agreement about the paternity of a child. In this case, additional legal steps must be taken in order to establish paternity.
