The Pennsylvania Adoption Search Request Form is a document that is used to initiate a search for a birth relative of an adopted person who was born in Pennsylvania. There are two types of Pennsylvania Adoption Search Request Forms: the Adult Adopted Search Request Form and the Non-Identifying Information Request Form. The Adult Adopted Search Request Form is used by adopters 18 years of age or older to search for their birth parents, and requires the adopted to provide information about their adopted name, date of birth, parents’ names, and other identifying information. The Non-Identifying Information Request Form is used by adopters or birth parents to obtain non-identifying information, such as medical information and family history. Both forms must be notarized and submitted to the Pennsylvania Department of Human Services Office of Adoption Services.
