The Oklahoma Verdict Form-Prior Convictions (First Stage) is a form used in Oklahoma courts to record the verdict of a jury in a criminal case, including any prior convictions. It consists of four parts: (1) the title, (2) the verdict section, (3) the prior convictions section, and (4) the signature section. In the title section, the form includes the name of the defendant, the court where the trial is taking place, the case number, and the date of the trial. In the verdict section, the jury is asked to mark if the defendant is guilty or not guilty, and if the defendant is found guilty, to specify the degree of the crime and the punishment. In the prior convictions section, the jury is asked to indicate if the defendant has any prior convictions and, if so, to list them. Finally, in the signature section, the jury members and the judge sign the form. There are two types of Oklahoma Verdict Form-Prior Convictions (First Stage): one for felonies and one for misdemeanors.
