The Initial Registration application requires the applicant to provide information about the pharmacy, including its name, location, type of business, and contact information for the owner and/or pharmacist. The applicant must also include a copy of their license or permit to operate a pharmacy in the state, proof of insurance, and a list of drugs and supplies to be sold.
