There are two different types of application forms: one for initial registration and one for transfer of ownership. The Initial Registration application requires the applicant to provide information about the pharmacy, including its name, location, type of business, and contact information for the owner and/or pharmacist. The applicant must also include a copy of their license or permit to operate a pharmacy in the state, proof of insurance, and a list of drugs and supplies to be sold.
