The New Jersey State Tax Case Information Statement (CIS-State) is a form issued by the New Jersey Division of Taxation that provides information related to a taxpayer’s pending audit or appeal with the State of New Jersey. The CIS-State form is typically issued to taxpayers as part of the audit or appeal process. The form contains the taxpayer’s name, address, Social Security number, and filing status, as well as contact information for the assigned auditor or appeal officer. The form also provides an explanation of the taxpayer’s rights and obligations relating to the audit or appeal process, including the right to appeal a decision. There are three different types of New Jersey State Tax Case Information Statement (CIS-State): (1) CIS-State Audit; (2) CIS-State Appeal; and (3) CIS-State Collection.
