New Hampshire Form FNP-5 Foreign Nonprofit Withdrawal is a form used by foreign nonprofit organizations to withdraw or terminate their registration with the New Hampshire Secretary of State. The form must be signed by a corporate officer of the foreign nonprofit, and submitted with a copy of the organization's articles of incorporation or other formation documents. The form must also include the name and address of the foreign nonprofit, the date of registration, and the name and address of the registered agent. There are two types of New Hampshire Form FNP-5 Foreign Nonprofit Withdrawal: Regular Withdrawal and Voluntary Withdrawal. The Regular Withdrawal is used when the foreign nonprofit is no longer actively conducting business in the state. The Voluntary Withdrawal is used when the foreign nonprofit voluntarily terminates their registration with the state.
