There are two types of North Carolina Notice of Correction of SSN: Form NC-NOC-SSN-R-01 and Form NC-NOC-SSN-R-02. Form NC-NOC-SSN-R-01 is used when the employee has a valid SSN, but the wrong SSN has been reported to the DES. It is also used to update the employee’s SSN in the DES records. Form NC-NOC-SSN-R-02 is used when the employee does not have a valid SSN, or the employee has a valid SSN but is unable to provide the SSN to the employer. This form is used to request a new SSN to be issued to the employee by the Social Security Administration. Both forms require the employer to provide the employee’s name, date of birth, and address.
