If you are a patient, family member, or another member of the general public, please utilize the complaint system link below, "File a Complaint". If you are a healthcare professional or law enforcement officer, you must contact the Board directly at 601-987-3079 to file your complaint.
You can make a complaint in writing by completing the Request for Complaint Resolution form, or you can call the customer service center at 1-877-882-4916. MDHS will contact you if more information is needed. Once all information is provided, MDHS will review your complaint and contact you within 3 business days.
All complaints must be submitted to the Commission office electronically, by fax, U.S. mail, or any mail service. The Commission does not accept hand delivered complaints to the Commission office. The Commission staff cannot provide legal advice or guidance in filling your complaint.
