Missouri Transmittal of Quarterly Wages Reported on Magnetic Media is a record of wages paid to employees that is sent to the Missouri Department of Revenue (FOR) on a quarterly basis. This record is submitted on a magnetic media, such as a diskette or a CD-ROM. The transmittal includes the employer's name, address, and Missouri tax identification number, as well as the names, addresses, and Social Security numbers of the employees. The total wages paid to each employee for the quarter and the amount of federal and state taxes withheld are also included. There are two types of Missouri Transmittal of Quarterly Wages Reported on Magnetic Media: Form MO W-3 and Form MO W-3M. Form MO W-3 is used to report wage information for the entire quarter, while Form MO W-3M is used to report wage information on a monthly basis. Both forms can be submitted electronically or by mail.
