Minnesota Appendix V: Affidavit of CLE Transfer is a document used by the Minnesota Supreme Court Board of Continuing Legal Education (CLE) to document a transfer of approved continuing legal education (CLE) credits from one attorney to another. The document is used when an attorney who has completed a CLE program wishes to transfer the credits to another attorney who did not attend the program. The document must be signed and notarized by both the transferring attorney and the receiving attorney. There are three types of Minnesota Appendix V: Affidavit of CLE Transfer: Transfer of Credit from Attorneys Licensed in Minnesota, Transfer of Credit from Attorneys Licensed in Other States, and Transfer of Credit from Attorneys Licensed in Other Countries.
