The Transition Plan — This plan is designed for individuals who are transitioning from school to work and may need additional support in order to make a successful transition. Services provided through this plan may include job coaching, job development, and other related services. 3. The Supported Employment Plan — This plan is designed for individuals who require more intensive supports in order to find and maintain employment. Services provided through this plan may include job coaching, job placement, and other related services. Additionally, the VRP provides individuals with access to a variety of resources and services, such as assessment and counseling, career development, and training and education.
