The Maryland Medical Certificate — Cessation of Disability (MMC-COD) is a document used to certify that an individual is no longer disabled. It must be completed by a licensed Maryland physician, and is used to inform the Maryland Department of Health (MPH) that an individual has recovered from their disability and is able to return to work. There are two types of MMC-CODs: Type I and Type II. Type I MMC-CODs are used for individuals who have been disabled for less than 12 months, while Type II MMC-CODs are used for individuals who have been disabled for more than 12 months. Both types of MMC-CODs must include the name and contact information of the physician, the date of the examination, the diagnosis, the relationship between the diagnosis and the disability, and any other relevant information. The MMC-COD must also be signed and dated by the physician.
