Massachusetts Voluntary Acknowledgement of Parentage (VAP) is a legal document used to establish a legal parent-child relationship between a father and a child when the father is not married to the mother. This document is signed by both the father and the mother and is then filed with the Massachusetts Registry of Vital Records and Statistics. The document is legally binding and creates a permanent record of parentage. There are two types of Massachusetts VAP: Unmarried Parent VAP and Assisted Reproductive Technology VAP. The Unmarried Parent VAP establishes legal parentage when the father is not married to the mother. The Assisted Reproductive Technology VAP is used to establish legal parentage when a sperm donor or surrogate is used to conceive a child.
