The Massachusetts Civil Cover Sheet is a form that is used to provide the court with basic information about a civil case. It is required by the Supreme Judicial Court for Civil Business (SJC-CIV) to be filed in all civil cases initiated in the Massachusetts court system. The form is completed by the Plaintiff and identifies the parties involved, the type of case, and the relief sought. The form also requests the name and address of the court to which the action is being brought, whether the action is being brought in a state or federal court, and the amount of damages requested. There are three types of Massachusetts Civil Cover Sheet: the General Civil Cover Sheet, the Summary Process Cover Sheet, and the Foreclosure Cover Sheet. Each of these cover sheets requests different information depending on the type of case being filed.
