The Massachusetts Notice of Address Change-Payment And-Or Notice Address is a form used to update a taxpayer’s address with the Massachusetts Department of Revenue. This form is used to notify the Department of Revenue when an individual taxpayer or business has changed their address, and it can also be used to update contact information for payments or notices. There are two different types of Massachusetts Notice of Address Change-Payment And-Or Notice Address forms: one for individuals, and one for businesses. The individual form must include the taxpayer’s name, Social Security number, new address, and signature. The business form must include the business name, Employer Identification Number, new address, and signature. Both forms must be signed and sent to the Massachusetts Department of Revenue in order to update the address change.
