Indiana Notary Complaint Form is a document used to file a complaint against a notary public in the state of Indiana. The form is administered by the Indiana Professional Licensing Agency (PLA). There are three types of Indiana Notary Complaint Forms: Notary Public Complaint Form, Notary Public Bond Complaint Form, and Notary Public Commission Complaint Form. The Notary Public Complaint Form is used to report any violations of the Indiana Notary Public Law or any other violations of professional conduct. The Notary Public Bond Complaint Form is used to report any violations of the Notary Public Bond or any other violations of professional conduct. The Notary Public Commission Complaint Form is used to report any violations of the Notary Public Commission or any other violations of professional conduct. All complaint forms must be completed and submitted to the Indiana Professional Licensing Agency.
