An Illinois Physicians Affidavit — Guardianship is a legal document used in the state of Illinois to appoint a guardian for an incapacitated adult. This document must be completed and signed by a qualified physician who has examined the incapacitated adult and can attest to the fact that the adult is in need of a guardian. There are two types of Illinois Physicians Affidavit — Guardianship forms available: an adult guardianship affidavit and a minor guardianship affidavit. Both forms require the physician to provide information about the incapacitated adult, the proposed guardian, and the medical basis for the guardianship. The completed affidavit must be notarized or filed in the court in order for the guardianship to be granted.
