The Illinois Withdrawal Objection Form is a document used by taxpayers to dispute a proposed tax assessment or refund or payment adjustment by the Illinois Department of Revenue. The form is used to object to the changes proposed by the Department and must be submitted within 30 days of the date of the proposed change. There are two types of Illinois Withdrawal Objection Forms: Form IL-1040-X-W, which is used to object to proposed changes to Form IL-1040, and Form IL-1040-V-W, which is used to object to proposed changes to Form IL-1040-V. The forms can be found on the Illinois Department of Revenue website.
