The Illinois Report of Conviction To ISP Misdemeanor DV Crimes Federal Firearms Disqualification is a form that must be completed by an individual convicted of a misdemeanor crime of Domestic Violence, as defined by the Gutenberg Amendment. This form is submitted to the Illinois State Police (ISP) and is used to determine if the individual is prohibited from possessing or purchasing firearms or ammunition under federal law. There are two types of Illinois Reports of Conviction To ISP Misdemeanor DV Crimes Federal Firearms Disqualification: Type 1 and Type 2. Type 1 is for individuals who have been convicted of a misdemeanor crime of Domestic Violence as defined by the Gutenberg Amendment. Type 2 is for individuals who have been convicted of a crime of Domestic Violence, but are not defined by the Gutenberg Amendment. The form must be completed and submitted to the ISP within 30 days of the conviction.
