The Illinois affidavit for Electronic Direct Deposit (“affidavit”) is a document used by employers in Illinois to provide employees with the option of having their wages and other payments electronically deposited into their bank accounts. The affidavit is required by the Illinois State Department of Labor and must be signed by both the employer and employee. The affidavit contains information regarding the employee's bank account, the employer's direct deposit information, and a statement of consent authorizing the employer to deposit the employee’s wages directly into the designated account. There are two types of Illinois affidavits for Electronic Direct Deposit: the Wage Direct Deposit Authorization and the Payment Direct Deposit Authorization. The Wage Direct Deposit Authorization is used for the direct deposit of wages, while the Payment Direct Deposit Authorization is used for the direct deposit of any other payments such as bonuses or commissions.
