The Idaho Transcript Designation & Ordering Form is used by Idaho students to request a copy of their official academic transcripts. The form allows students to designate who should receive the transcript, the delivery method, and any special instructions. There are two types of Idaho Transcript Designation & Ordering Forms: one for high school students and one for college students. The high school version allows students to provide information about the school they attended, the year they graduated, and any additional information that may be necessary for the transcript request. The college version requires students to provide their college name, student ID number, the term or semester they attended, and their major. Both forms provide space for students to list the recipient's name and address, as well as a signature line for the student to authorize the request.
