The Hawaii Statement of Amendment is a statutory form that is used to notify the State of Hawaii of changes to an existing business entity. It is required for corporations, limited liability companies (LCS), general partnerships, and limited partnerships. The Hawaii Statement of Amendment can be used to amend the formation documents for the business entity, such as the articles of incorporation or certificate of formation, operating agreement, or partnership agreement. It can also be used to make changes to the name, location, or purpose of the entity; add or remove members or managers; or appoint or remove an agent for service of process. There are three types of Hawaii Statement of Amendment: Certificate of Amendment, Articles of Amendment, and Statement of Amendment.
