The District of Columbia General Forms (SEB)-Precise — Change of Address (Legal) is a form used to update the address on an existing court case in the District of Columbia. The form allows for the petitioner to provide their new address and contact information, as well as to provide a statement of the reason for the change in address. It can be used to update the address of any party to a case. The form is available in two versions: one for a private individual, and one for an attorney or law firm. Both versions must be completed and signed by the petitioner in order to be accepted by the court.
