The Colorado Chapter 13 Long Form Fee Application is a form used in Colorado by attorneys to document, calculate, and request court-approved fees associated with filing a Chapter 13 bankruptcy petition. The form is used to apportion the fees among the debtor, creditors, and the court. There are three types of Colorado Chapter 13 Long Form Fee Applications: the Initial Fee Application; the Amended Fee Application; and the Final Fee Application. The Initial Fee Application is used to make the initial request for fees, the Amended Fee Application is used to make changes to the initial request for fees, and the Final Fee Application is used to make the final request for fees. The form must be completed and signed by the debtor, the debtor's attorney, and the Chapter 13 Trustee.
