Statement of Parolee or Mandatory Releasee Concerning Appointment of Counsel Under the Criminal Justice Act The Arizona Statement of Parolee or Mandatory Released Concerning Appointment of Counsel Under the Criminal Justice Act is a form issued by the Arizona Department of Corrections which is used to provide the parolee or mandatory released with an opportunity to appoint counsel of their choice to provide legal representation under the Criminal Justice Act. This form also informs the parolee or mandatory released of their right to have counsel appointed and their responsibility to pay for counsel if they choose to do so. This form is used to ensure that the parolee or mandatory released is aware of their rights and obligations under the Criminal Justice Act. There are two types of Arizona Statement of Parolee or Mandatory Released Concerning Appointment of Counsel Under the Criminal Justice Act: 1.
