The Arizona Statement of Parolee or Mandatory Released Concerning Appointment of Counsel Under the Criminal Justice Act is a form issued by the Arizona Department of Corrections which is used to provide the parolee or mandatory released with an opportunity to appoint counsel of their choice to provide legal representation under the Criminal Justice Act. Form 3.5-3 is used by the parolee or mandatory released to request the appointment of counsel, while Form 3.5-4 is used by the parolee or mandatory released to make a request for counsel and to pay the cost of counsel. Both forms must be completed and signed by the parolee or mandatory released, and the form must be submitted to the Arizona Department of Corrections for processing.
