The Arizona Data Sheet — Judicial Consent to Abortions for Minors is a form used to allow a minor to obtain an abortion without the written consent of a parent or guardian. There are two types of Arizona Data Sheet — Judicial Consent to Abortions for Minors: 1) Judicial Bypass and 2) Consent of Parent or Guardian. The Judicial Bypass form is used when a minor wishes to obtain an abortion without the written consent of a parent or guardian, and the minor has obtained a court order granting permission for the abortion. The Consent of Parent or Guardian form is used when the minor has obtained written consent from a parent or guardian. Both forms must be filled out and signed by the minor and the appropriate parties prior to the abortion procedure.
