The Alaska Adversary Proceeding Cover Sheet is a document used to initiate an adversary proceeding in the US Bankruptcy Court for the District of Alaska. The cover sheet provides information about the proceeding and the parties involved, including the debtor, creditor, and the attorney or other representative filing the proceeding. It also includes the name and number of the case, the date the adversary proceeding was filed, and the type of adversary proceeding being initiated. There are two types of Alaska Adversary Proceeding Cover Sheet. The first is the “Adversary Complaint and Summons” Cover Sheet, which is used to initiate a complaint against the debtor. The second is the “Motion and Order to Show Cause” Cover Sheet, which is used to initiate a motion to compel certain action from the debtor.
