1) Submit Official Shot Record. Once you receive your Collin College student email, submit documentation directly to Med+Proctor. Select the "Register" button and enter your student email address.
Complete required personal information, download the immunization certificate, and follow the directions provided. Upload a copy of your immunization forms to your Med+Proctor account. Make sure your forms are complete and legible.
BEFORE the application deadline, send official transcripts from all previous colleges attended to: Respiratory Care Program Director --Office H207, Collin College Central Park Campus 2200 West University Drive, McKinney, Texas 75071. If you are NEW to Collin, please submit transcripts to the Admissions Office first.
Students intending to use third party funding should present the signed authorization and release form and email it to cashier@collin or present it at any cashier office location.
You may upload your records to the Student Health Immunization Record Upload Center B. You may bring a copy of your immunization record to Red Raider Orientation to give to SHS personnel during registration. We will keep the record.
Petition for Degree / Certificate Under Student Quick Links, select "Apply to Graduate." Students may complete a "Petition for Degree / Certificate" at any of our Student Enrollment and Services offices or you can download a form from the Admission Forms section of our web site.
On the portal home page, click on “Immunization/TB Upload” on the left menu bar. c. On the “Immunization/TB Upload” page, click on the “Add Immunization Record” button and navigate to your scanned completed document (e.g., Immunization records, TB Health Assessment Form, etc…).
Log onto the site with your MSU NetID and password. Click the "Complete your Immunization Self-Report Form" button. Fill out the form. You may need to contact your primary care physician, previous schools, or parents to find the required information.

Transcript requests will only be accepted online, through the mail, or in person. No faxed, emailed, or phone requests will be accepted.
