The Care caregiver form with 504 in Minnesota is designed to outline the terms of employment between a caregiver and a client. This form provides essential details regarding the nature of services offered by the caregiver, including assistance with daily living activities, medication scheduling, and mobility support. It establishes a clear working relationship, emphasizing the caregiver's role as an independent contractor rather than an employee of the client. The form includes provisions for adjusting schedules, requiring a 48-hour notice for changes, and can be terminated with two weeks' written notice from either party. It also highlights the need for both parties to consider consulting a lawyer before signing to ensure mutual understanding of the terms.
