Caregivers providing care to “high-need” older adults—those who have at least two self-care needs or dementia—are more likely to help with a wide variety of tasks, including helping with chores, helping the older adult get around the house, keeping track of medications, and making medical appointments.

Isolation poses severe risks for mental and emotional well-being. Loneliness is a common experience for elderly people without family or friends nearby. Without companionship or daily engagement, they may struggle with motivation, leading to poor self-care habits such as neglecting exercise, nutrition, or hygiene.
Depending on the medical assessments conducted by the Department of Elder Affairs, seniors may be eligible for a wide range of supports, such as nursing home care, dental care, prescription drugs, transportation and homemaker services.
They empower older adults and families to find appropriate services. They also facilitate family support, provide counseling and direct services, and coordinate care delivered through professional systems.
The AHCA 5000–3008 form is used by the Comprehensive Assessment and Review for Long-Term Care Services (CARES) Program to help determine medical eligibility for Medicaid Waiver programs. This form must be signed by a licensed physician, physician assistant, or advanced practice registered nurse.
If you become concerned, go over and knock on the door. Listen and offer your support. Get help from Adult Protective Services if you are concerned an adult may be self-neglecting. Call 911 if the person needs immediate medical attention or is in danger of immediate harm.
