Description

The Caregiver Form Application with VA in Arizona is designed to formalize the relationship between a caregiver and their client, specifying the terms of service and responsibilities. This agreement outlines the types of assistance caregivers will provide, including daily living activities, medication management, and mobility support. It ensures that care is tailored to the client's preferences with a mutually agreed-upon schedule, which requires a 48-hour notice for any changes. The agreement can be terminated by either party with two weeks' written notice, providing flexibility for both caregivers and clients. Importantly, the document emphasizes that caregivers operate as independent contractors, protecting the client's legal interests and limiting liability for negligence. Attorneys, partners, owners, associates, paralegals, and legal assistants will find this form useful for establishing clear expectations in caregiver-client relationships, as it serves as an essential tool for safeguarding both parties' rights and outlining the scope of care. The form also encourages consultation with a lawyer to ensure all parties fully understand the obligations involved, promoting transparency and trust in caregiver services.
