The parties may agree to a different performance. This is called an accord. When the accord is performed, this is called an accord and satisfaction. The original obligation is discharged. Participating physicians, professional providers, ancillary and facility providers are requested to submit claims electronically to Blue Cross and Blue Shield of Texas (BCBSTX) within 95 days of the date of service, or by using the standard CMS-1500 or UB04 claim form.

File electronically, as usual. File the claim in its entirety, including all services for which you are requesting reconsideration. BCBSTX will adjust the original claim. The corrections submitted represent a complete replacement of the previously processed claim.
