The DS-11 Application Form for New US Passport is an application form that should be used when you are applying for a new US passport or when you are replacing an expired US passport that was issued to child under the age of sixteen; Or for a lost passport.
If one parent is not available, the non-applying parent must complete the form DS-3053 . This form must be notarized, and the identification that was presented to the notary must be copied, back and front, and included with the form. We require the original of the DS-3053 to adjudicate the passport application.
General Declaration (B11) Where an applicant's circumstances require further explanation, passport interviewing officers may advice the applicant or parent (or person with parental responsibility) to complete a general declaration known as a Form B-11 General Declaration by Passport Applicant.
This form is used when the name that the passport applicant uses is (1) substantially different from the name on the passport applicant's citizenship evidence or (2) the passport applicant's name was not acquired by marriage or a court order.
The main difference is that applications using the DS-11 form are always in person. The DS-11 form is used for first-time applications, applications for citizens under 16, and if the applicant does not fully satisfy the requirements mentioned above. DS-82 is required for passport renewals by mail.
