Most applicants who are age 1617 must apply in person using Form DS11. Passports for applicants 16 and older are valid for 10 years.Use if you are applying for the first time, for your child who is under age 16, or you don't meet our requirements to renew your passport. If you are age 16 or older, your passport is valid for 10 years. Your child's passport (age 0-15) is valid for 5 years. All legal guardians must be present or submit a consent form (DS-3053). Original Valid ID is required for applicants 16 years and older. Minors: For applicants under 16 years of age both parents must be present or provide official consent.
