The letter must contain the following: The statement: "Please remove my name and address from all mailing lists. This is a request that my personal information is removed from all of your mailing lists for unsolicited mail.To opt out permanently: Go to optoutprescreen. Com or call 1-888-5-OPT-OUT (1-) to start the process. Please remove my name and address from your mailing list. I want to reduce the amount of unsolicited mail I receive. If this request is not sufficient for removing my name or those of my listed family members, please notify me promptly of any other procedures I need to follow. If you would like, you can also request a form that will wipe your name PERMANENTLY from their mailing lists. Other mail, cross out the address, write "return to sender", and write "remove from mailing list" in marker. Then drop it in a mail box.
