A formal Medical Exemption may be completed for persons with certain medical conditions and/or contraindications (permanent or temporary) to certain vaccines. A letter from the student's physician or the official State of Florida Medical Exemption form must be submitted to the SHCC.

How you can apply. If you're entitled to a certificate because of your medical condition, speak to your GP or doctor. They'll give you an application form. You'll get a certificate in the post within 10 working days of us receiving your application.
