Form Interrogatories are a list of questions on a court form. Form interrogatories are contained in Judicial Council Form FL145.This packet provides general guidance about asking written questions, called "Interrogatories," to another party in the case. You need to file a Request for Additional Time to answer the Interrogatories due to their length and depth. Each interrogatory shall be full and complete in and of itself. You should respond to the interrogatories in a timely manner i.e. How do I fill this out? When making interrogatories, you must create written questions, label them as "interrogatories," and demand the other party answer them in a letter. 1.
