We are sending you this letter because you were in the MEDICAL CENTER NAME, ADDRESS, between DATE and DATE. During this time, you were identified to have been exposed to another individual in MEDICAL CENTER NAME who tested positive for Coronavirus Disease 2019 (COVID-19) while this individual was infectious.
In ance with guidance from the CDC, if you test positive for COVID-19 you should: Isolate for 5 days regardless of vaccination status. Only leave isolation after 5 days if you have no symptoms or your symptoms are improving, including at least 24 hours without a fever.
If you test positive, you are strongly encouraged to isolate (stay home and away from other people) for at least 5 full days, to prevent spreading the disease to others. Isolation is recommended even if you have been vaccinated or have had COVID-19 before.
You are considered contagious from two days before your symptoms begin until 10 days after your symptoms began. If you have no symptoms, you are considered contagious beginning two days before your test sample was collected and until 10 days after your test sample was collected.
You should isolate for at least 5 days counting from the day you began feeling sick (Day 0 is the day you began feeling sick; Day 1 is the next day). If you have no symptoms, then isolate for 5 full days after the day you tested positive (Day 0 is the day you took your positive test; Day 1 is the day after).
You may apply for a COVID-19 recovery certificate if you have had a positive COVID-19 test result between the last 10 days and 180 days. You are able to apply and upload your evidence prior to 10 days, but your certificate will only be sent to you on day 10.
