FAQ

Withdrawals from retirement plans are reported on Form 1099-R. There are a few variations of the form, Form CSA 1099-R, Form CSF 1099-R, and Form RRB-1099-R, but they all report retirement account distributions.

You may be able to get the penalty waived if you missed taking your RMD due to an illness, mental incapacity, or bank error. You'll calculate that penalty in Part 9 of Form 5329. To request a waiver, enter “RC” (for reasonable cause) and the amount of shortfall you want waived on the dotted line next to Line 54.

Do I have to attach Form 5498 to my income tax return? No. You aren't required to do anything with Form 5498 because it's for informational purposes only.

If you aren't required to file an income tax return, you can complete and file Form 5329 by itself at the time and place you would be required to file Form 1040, 1040-SR, or 1040-NR. Remember: you can't file it electronically if you file it by itself.

Tax reporting and withholding Note: You are required to report your withdrawals and file Form 8606 with your tax return, even if you take a nontaxable distribution that is equal to or less than your total contributions to all of your Roth IRAs.

File Form 1099-R, Distributions From Pensions, Annuities, Retirement or Profit-Sharing Plans, IRAs, Insurance Contracts, etc., for each person to whom you have made a designated distribution or are treated as having made a distribution of $10 or more from profit-sharing or retirement plans, any individual retirement ...

When you take a distribution from your 401(k), your retirement plan will send you a Form 1099-R. This tax form shows how much you withdrew overall and the federal and state taxes withheld from the distribution if applicable.

You will receive a Form 1099-R when you make a withdrawal from a IRA, 401(k) or other retirement account. This form includes information such as: the amount you withdrew, how much is taxable (if that was determined), any taxes that were withheld, and a code that shows what type of distribution it was.

Note: The W-4 form 2024 steps are the same as the W-4 form 2025 steps. Step 1: Enter your personal information. Step 2: Account for all jobs you and your spouse have. Step 3: Claim your children and other dependents. Step 4: Make other adjustments. Step 5: Sign and date your form.
