Withdrawals from retirement plans are reported on Form 1099-R. There are a few variations of the form, Form CSA 1099-R, Form CSF 1099-R, and Form RRB-1099-R, but they all report retirement account distributions.
You may be able to get the penalty waived if you missed taking your RMD due to an illness, mental incapacity, or bank error. You'll calculate that penalty in Part 9 of Form 5329.
