Large 401(k) plans (those with more than 100 participants) and some small 401(k) plans that don't meet DOL waiver requirements must file an independent audit report with their Form 5500.

However, many employers are unaware that they may also have a 5500 requirement for their health and welfare plans, possibly including health, dental, vision, life insurance, short-term and long-term disability, and Health Reimbursement Arrangement (HRA) plans.

Expense reimbursements under HRAs for qualified medical expenses are not reportable to the IRS. No other distributions from the HRA are allowed and therefore, there is no IRS reportable event.

Under EFAST2, you must electronically file the 2023 Forms 5500 and 5500-SF using EFAST2's web-based filing system or you may file through an EFAST2-approved vendor. The 2023 Form 5500-EZ can also be electronically filed using the EFAST2 filing system or filed on paper with the IRS.

You do not have to file Form 5500-EZ for the 2023 plan year for a one-participant plan if the total of the plan's assets and the assets of all other one-participant plans maintained by the employer at the end of the 2023 plan year does not exceed $250,000, unless 2023 is the final plan year of the plan.

The plan number is a three-digit number that is assigned to the plan by the plan administrator or plan sponsor. This three-digit plan number, in conjunction with the employer's nine-digit employer identification number (or EIN), creates a unique 12-digit number that is used by the DOL to identify the plan.
