Birth records can only be amended if the certificate holder is alive. Any posthumous amendments require a court order. Any field that allows a change to be made with an affidavit can only be changed one time. All other changes require a court order.

Birth Records are available from 1905 - present. Certificates may be ordered online at vitalrecords.utah. Please read this application carefully. It is a criminal violation to make false statements on this application or to fraudulently obtain a vital records certificate.

Legally the hospitals have 10 days to register the birth with the State Office of Vital Records, but it can take another 1-2 days before we have access to it for issuing. Once we have access to the record, it is available within minutes when requested in person.

If the father isn't listed on the birth certificate, he can still pursue custody by establishing paternity legally. However, his behavior, financial status, and mental state will be considered.

The unmarried father will be permanently precluded from claiming any rights unless he has previously established that he is entitled to notice of an adoption proceeding. See Utah Code section 78B-6-110(3).
