NYS-45, Quarterly Combined Withholding, Wage Reporting, and Unemployment Insurance Return. All employers required to withhold tax from wages must file Form NYS-45, Quarterly Combined Withholding, Wage Reporting, and Unemployment Insurance Return, each calendar quarter.
The amount deducted appears on the employee's paystub and the total amount deducted annually can be found on Form W-2: Wage and Tax Statement. Employers send W-2s to their employees each year to help them file their annual income tax returns.

Form NYS-45 is much like the state's version of federal form 941 for quarterly reporting of tax and income withholding, except the NYS version also adds information for unemployment insurance.

A business may register for a Georgia withholding tax account number through the Georgia Tax Center (GTC), a secure electronic self-service portal. After the online submission, employers will receive an email within 15 minutes of their assigned withholding tax account number and deposit tax frequency.
