A complaint is a sworn allegation charging the accused with the commission of an offense. Article 45A. 101 of the Code of Criminal Procedure lays out the requirements for a complaint.
Most complaints are made by the person who received the service they are complaining about, or by someone who has been affected by something you have or have not done, or a decision made by your organisation.
You can make a complaint by mail or fax, but HHS prefers that people submit online via the Texas Unified Licensure Information Portal (TULIP) or call 1-800-458-9858 so we can get all of the information right away to investigate and resolve a complaint.
