You are not permitted to read newspapers or other materials in the courtroom. You are not permitted to bring food or beverages into the courtroom.

If you are selected to serve on a trial, you must remain for the duration of jury selection and the trial. Once your service is completed, you will not be eligible to be selected again for three years.

You may request to be excused from jury service you if fall into one of the categories below. You are 75 years of age or older. You have served as a juror in the same county within the last three years. You believe that you will suffer a severe financial hardship. You have a medical inability to serve as a juror.

Failure to return the form at this time will result in the issuance of a summons for you to appear in court to complete the questionnaire. Thereafter, the penalty for failure to respond may include a fine of not more than $1000.00 or imprisonment of not more than 3 days, or both pursuant to Title 28, U.S.C.

Jurors reporting for service should wear clothing appropriate for an appearance in court. Shorts, t-shirts, uniforms or clothing containing statements or offensive symbols are not permitted. All hats must be removed when in a courtroom. Comfortable clothing is also advisable.
