If you were married in Maryland, you can request a new marriage record that reflects your new name by filing a Request for New Marriage Record (CC-FM-072) in the circuit court of the county that issued the record.
If you took a new name when you were married and want to take back the name you used before the marriage, use a form titled Motion for Restoration of Former Name, or form CC-DR-097. Your reason for taking back your former name may not be illegal, fraudulent, or immoral, or to hide from creditors.
If both parents do not consent to the name change, then you may ask the court to decide. You need to show the court that the name change is in the child's best interests, and that the circumstances of your case are extreme.

Your Angel's name comes to you ultimately from “Source” or God. The Source includes your Higher Self, your personal Guardian Angel(s) and the Archangels and other beings of light, such as Ascended Masters. You can sit in your meditation for as long as you like. You may hear your guardian angel's name as an intuitive thought or an audible sound, or you may be guided to write in your journal. If you don't receive the name in your meditation, you can trust that it's coming.
The traditional teaching of the Church is that you should not name your Guardian Angel because to name something implies you have some sort of authority over it. It is better to ask your angel what their name is in prayer and see if they reveal it to you.
Examples of documents needed for each child may include but are not limited to : birth certificate, official custody or guardianship papers, clothing and shoe sizes, and a toy wish for each child. No private information for you or your Angel is shared with the public.
Can I ask my guardian angel his name or can I name my angel myself? Asking for the name of one's guardian angel is forbidden by the Church.
The Lord has not revealed whether one specific angel is assigned to watch over each person, but you can be assured that divine protection and comfort are available. If you exercise faith, you will have God's help, including angels sent to strengthen and comfort you and give you courage to do what is right.
