These are forms designed by banks and credit unions to provide the payee's name, address, and account details. Direct deposit authorization forms are available online, usually through your payee's online account access, or from a local bank branch.
The Guidelines set a minimum child support amount of $91.00 per child per month.
If the check must be payable to the custodial parent, please include the SDU as the co-payee. For example, please make the check payable to “custodial parent and/or the state SDU.” Sending payments electronically to state SDUs is another way to ensure payments arrive quickly and safely.
However, if you are currently supporting another child or a spouse (for example, if you have remarried and had another child), the court can order that up to 50% of your wages be withheld for child support. The amount withheld from your check – about a third of your wages – is well within these limits.

Submit your completed form to your employer, their payroll company, or their HR department. You may need to scan your printed copy, or they may ask you to mail it or deliver it in person. Or you may be able to submit the form by email. This will depend on your employer's preferences.
If you want to use the direct deposit option, contact you local Domestic Relations office or Pennsylvania SCDU. Either location will provide the appropriate form for you to complete. Completed Direct Deposit forms must be sent to PA SCDU for processing.
