Sometimes you may make a mistake on your initial Form 941 that you need to correct. You'll then use Form 941-X to amend the original filing. This amendment form can also be used to claim credits retroactively if you are eligible but failed to claim it for the applicable quarter.

Form 941-X is used to correct any errors made on an IRS Form 941 form that's already been filed. This would include business information, such as an EIN or business address, and any amounts indicated on the form (employee wages, income tax withheld from wages, etc.).

When you discover an error on a previously filed Form 941, you must: Correct that error using Form 941-X; File a separate Form 941-X for each Form 941 that you're correcting; and. Generally, file Form 941-X separately. Don't file Form 941-X with Form 941.

You can file Form 1040-X, Amended U.S. Individual Income Tax Return electronically with tax filing software to amend your Form 1040, 1040-SR, 1040-NR, or 1040-SS/PR for the current or two prior tax periods. If amending a prior year return originally filed on paper, then the amended return must also be filed on paper.

If you discover an error on your previously filed Ohio return, use Form IT-1040 to file an amended return. This applies for both the 2023 and past tax years. However, note that Form IT-1040 is specific to Ohio and cannot be used for federal tax amendments. For federal errors, please refer to IRS Form 1040X.
