This Revised Uniform Anatomical Gift Act Donation form pursuant to state statutes designates the specific body parts and organs an individual wishes to donate at the time of death. Any individual eighteen years of age or older and of sound mind may make an anatomical gift for any statutorily specified purpose by execution of a document of gift or by will.
