This form is a Renunciation and Disclaimer of Life Insurance or Annuity Contract proceeds. This form is for a beneficiary who gains an interest in the described proceeds upon the death of the decedent, but, pursuant to the North Carolina General Statutes, Chapter 31B, decides to renounce his/her interest in the proceeds. The beneficiary also attests that the disclaimer will be filed no later than nine months after the death of the decedent in order to secure the validity of the disclaimer.
